As part of Live Active Leisure's Service Level Agreement with the Scottish Football Association, we are required to register all football participation on the Scottish FA's Football Administration System which collates participation figures regionally & nationally. Please can you confirm if you give consent for the following details (Name, Date of Birth, Postcode) to be added to the Scottish FA Football Administration System for reporting purposes only.
